- Revoke Authorizations: Withdraw permission for us to use or disclose PHI, except where already acted upon.
- Receive a Paper Copy: Request a printed copy of this Notice at any time.
3. How We May Use & Disclose Your PHI
We may use or disclose your PHI for:
- Treatment: To share with other healthcare providers involved in your care.
- Payment: To bill your health insurance or other payers.
- Healthcare Operations: To review treatment quality and train staff.
- Business Associates: To vendors who provide services (e.g., labs, pharmacies), under HIPAA-compliant contracts.
